At Forest Grove Veterinary Clinic, behavioural services are led by Lynnsey, our Veterinary Behaviour Technician. Lynnsey was the first RVT in Canada to earn her VTS-Behaviour designation. She also holds her Low Stress Handling Silver Certification and Fear Free Professional ELITE Certification.
Our goal is to help pets and their families feel safer, more confident, and better supported through a thoughtful behaviour consultation process.

For Clients
Requesting a Behaviour Consultation
Referral Form Completion
Before a behavioural consultation can be booked, your referring veterinarian must complete the Behaviour Consultation and Referral Form.
Behaviour Consultation and Referral FormSubmit Your Pet’s Behaviour Questionnaire
Once the referral form has been completed, please request a behavioural consultation for your pet by completing the correct form below. This helps our team collect your preferred appointment day and time and gives Lynnsey important background information about what is happening at home, since you are your pet’s primary caregiver.
Clinic Follow-Up
After the appropriate forms are submitted, our team will review the information and follow up with next steps for scheduling.

Please complete the Veterinary Behaviour Consultation Referral Form so our team can review the patient’s medical history, current concerns, and referral details before the client is scheduled.

Review & Scheduling
Once both the referral form and client questionnaire are received, Forest Grove Veterinary Clinic will review the information and follow up regarding scheduling.
