Behaviour concerns can feel overwhelming, confusing, and sometimes even scary for pet owners. Whether your pet is struggling with fear, anxiety, reactivity, aggression, inappropriate elimination, or you are a first-time pet owner looking for guidance, our team is here to help you better understand what your pet may be trying to communicate.

At Forest Grove Veterinary Clinic, behavioural services are led by Lynnsey, our Veterinary Behaviour Technician. Lynnsey was the first RVT in Canada to earn her VTS-Behaviour designation. She also holds her Low Stress Handling Silver Certification and Fear Free Professional ELITE Certification.

Our goal is to help pets and their families feel safer, more confident, and better supported through a thoughtful behaviour consultation process.

Referral Form Completion

Before a behavioural consultation can be booked, your referring veterinarian must complete the Behaviour Consultation and Referral Form.

Submit Your Pet’s Behaviour Questionnaire

Once the referral form has been completed, please request a behavioural consultation for your pet by completing the correct form below. This helps our team collect your preferred appointment day and time and gives Lynnsey important background information about what is happening at home, since you are your pet’s primary caregiver.

Clinic Follow-Up

After the appropriate forms are submitted, our team will review the information and follow up with next steps for scheduling.

Client Completes Their Questionnaire

After the referral form is submitted, the pet owner will need to complete the appropriate canine or feline behaviour consultation request form.

Review & Scheduling

Once both the referral form and client questionnaire are received, Forest Grove Veterinary Clinic will review the information and follow up regarding scheduling.
